We automatically use the name of your form as a form header to let people know where they are. If this isn't what you want you can turn it off.
Open a form from Forms and go to Settings > Display settings. The Show header switch toggles the form header on or off. Save your form when you’re done.
You have control over the design of other parts of your form as well. Why don’t you try creating a theme for your form?
